Scope of Services The Oak Tree Rise Scheme features eight fully independent, accessible bungalows situated in the heart of Northampton (postcode area NN1 1ED), creating a supportive yet autonomous living space for residents. Successful providers will be responsible for comprehensive on-site management, including:

  • Specialist Daily Support: Round-the-clock assistance with personal care, daily living skills, emotional regulation, and behavioral interventions, customized to each individual’s care plan.
  • Health and Wellbeing Integration: Coordination with multidisciplinary teams for medical appointments, therapy sessions (e.g., occupational, speech, and sensory therapies), and crisis prevention strategies.
  • Community Engagement: Facilitating social activities, skill-building workshops, and connections to local resources to promote social integration and reduce isolation.
  • Risk Management and Safeguarding: Robust protocols for handling complex behaviors, medication administration, and emergency responses, ensuring compliance with all regulatory standards.
